How Referrals Work

Submit a referral

Complete the form below with the participant’s NDIS plan details and contact information — anyone can refer, including support coordinators, GPs, other allied health providers, family members, or the participant themselves.

We review your referral

Our team checks the referral against the relevant NDIS support category and the services you’re seeking.

We contact you directly

Within 1–2 business days of receiving a referral, we’ll reach out to the participant or their nominee to confirm details and answer questions.

Your first appointment is booked

Once details are confirmed, we’ll schedule an initial assessment at our Balcatta clinic or, for eligible services, via telehealth.

Who Can Make a Referral?

We are an NDIS registered provider (Organisation ID 4053531602). Anyone can refer: participants, families, GPs and support coordinators.

You don’t need to be a medical professional to refer someone to AF Therapy Services. We accept referrals from support coordinators, GPs and specialists, other allied health providers, family members and carers, and self-referrals from NDIS participants.
